Definition
Lock Out Tag Out is a safety procedure used in industry to ensure that dangerous machines are properly shut off and not able to be started up until the completion of maintenance or repair work.
Aim:
- Isolation of dangerous power source
- Notification of work in progress
- Notification of workers performing LOTO
- Fastener/key (E.g Hasp Key)
- Pad lock Tag (E.g LOCKOUT)
SITUATION REQUIRING LOCK OUT TAG OUT
- Personnel doing servicing, maintaining or repairing any equipment where electrical hazards exists
- Operators working with circuits and electrical equipment with the risk of electrical shock or explosion
- Operators performing works near other hazardous /dangerous energy which may cause stored energy (electrostatic energy stored in capacitors) to be released unintentionally and causing injury
- Others power source is to be switched off
- Prepare for work cessation
- Ensure equipment machine are switched off
- Install LOTO at the main switches
- Perform the tests to verify the effectiveness of the LOTO
- Only those authorized may open the lock
- Adhere to checklist
- Employees performing the work with the electrical equipment themselves
- Use right LOTO for the right equipment
