- The object or load to fall.
- Crane moves beyond the top of the head.
- Strap damaged.

General Prevention:
- Inspect and test all equipment and make sure it is in safe operating limits before lifting.
- Make sure the work area is partitioned properly - DO NOT walk or stand under a hanging load.
- Crane operator and a flagman, shall be of a competent and well-trained people.
- Avoid "over-winding" as handed rope fell away from the drum.
- Ropes must be fastened properly.
- Check the hook & pin from wear and damage.
- Do not use non standard hooks or home made screws.
